Flextight Precision II scanner. It has been used mainly by staff for the last couple of years. Perfect working order. Scan quality seems pretty much identical to the later model 949 we had here for a while apart from being much slower. 
It runs off scsi so you need an OLD mac similar to the one pictured to run it. (About $100-$200 on ebay) Includes holders for 35mm, 120 and 4x5" formats.
